I, as the parent/guardian of the above named camp registrant ("my child"), do hereby grant permission for my child to participate in LADY PIRATE VOLLEYBALL CAMP and acknowledge the fact that my child is physically able to participate in camp activities. I hereby release the camp and its camp employees, Lubbock-Cooper ISD, the LCISD Board of Trustees, and LCISD employees from all claims from injury or illness which may be sustained by my child during the course of LADY PIRATE VOLLEYBALL CAMP. Should medical treatment be necessary, I authorize the camp director or designee to select hospital facilities and/or physician(s) and authorize medical treatment for my child.
